Nous, soussignés, sommes des cliniciens issus de divers domaines tels que la psychiatrie, la psychanalyse, la psychologie, le counseling clinique et le travail social, résidant et exerçant en Amérique du Nord, en Amérique du Sud et en Europe. Nous vous écrivons pour exprimer notre profonde inquiétude face à la décision du C.I.U.S.S.S. de fermer Le 388 (Centre de traitement pour jeunes adultes psychotiques) à Québec. Cette décision représente un risque majeur pour le paysage de la santé mentale, tant à l’échelle locale qu’internationale, et aura des répercussions négatives sur les personnes les plus vulnérables ayant besoin de ces services.
Le 388 est une institution essentielle, à la pointe du traitement de la psychose. Son intégration de la psychanalyse dans un cadre communautaire et son approche interdisciplinaire et multimodale constituent une véritable source d’inspiration pour nombre d’entre nous. À Chicago, Boston, New York, Los Angeles et bien au-delà, nous avons étudié et continuons de nous référer au modèle de traitement développé au 388 pour orienter nos propres pratiques cliniques auprès des patients souffrant de psychose. En tant que cliniciens en première ligne de la santé mentale, nous reconnaissons la valeur unique de l’approche du 388, qui répond aux besoins de patients souvent laissés pour compte par les structures existantes. Ce traitement offre une rare et précieuse opportunité pour les personnes en souffrance : non seulement gérer leurs symptômes, mais aussi explorer leur humanité, s’engager dans une transformation profonde et trouver de nouvelles manières de contribuer à la société. Ce centre n’est pas simplement un établissement de soins — c’est un lieu de refuge, où des individus confrontés à d’immenses difficultés peuvent se reconstruire, comme en témoignent de nombreux patients. Sa fermeture signifierait le démantèlement d’un modèle de soins profondément enraciné et irremplaçable, qui répond à la complexité de la psychose comme peu d’autres établissements peuvent le faire.
Beaucoup d’entre nous ont parcouru des centaines, voire des milliers de kilomètres pour découvrir l’approche psychanalytique du 388 et participer aux séminaires de formation proposés par le GIFRIC. Cette formation a été essentielle à l’évolution de nos pratiques cliniques. L’enseignement et le leadership des analystes Lucie Cantin, Danielle Bergeron et Willy Apollon ont eu un impact considérable, inspirant des organisations et des praticiens bien au-delà du Québec. La rigueur intellectuelle et éthique qui caractérise le 388 a marqué des cliniciens, universitaires et éducateurs non seulement en psychanalyse, mais aussi en psychiatrie, en counseling clinique, en psychologie clinique, en anthropologie et en travail social. Le 388 a été un centre de recherche et une clinique exemplaire, avec des résultats démontrés en matière de traitement et de réintégration à long terme des jeunes adultes psychotiques dans la communauté, en tant que membres actifs de la société. Il est difficile de surestimer l'impact unique du travail effectué au 388 sur l'émergence de nouvelles idées dans le domaine du traitement et de la recherche en santé mentale.
La fermeture du 388 ne signifierait pas seulement la disparition d’un modèle de soins exceptionnellement efficace, mais représenterait également un recul significatif pour la communauté mondiale de la santé mentale, qui considère cette institution comme une référence en matière d’innovation et d’excellence clinique. Nous implorons les décideurs d’engager un dialogue avec les parties prenantes, notamment les cliniciens, les patients, leurs familles et la communauté, afin d’explorer des alternatives qui permettraient de préserver l’inestimable travail accompli au 388. En garantissant la poursuite de ses activités, vous préserverez un véritable espoir pour les personnes souffrant de psychose et favoriserez l’avancement des soins en santé mentale à l’échelle internationale.
Travaillons ensemble pour sauvegarder et étendre l’héritage du 388, plutôt que d’en faire un exemple regrettable d’un potentiel perdu. Nous vous exhortons à reconsidérer cette décision.

Dear Minister Carmant,
We, the undersigned, are clinicians from fields including psychiatry, psychoanalysis, psychology, clinical counseling, and social work, who live and work in North America, South America, and Europe. We are writing to express our profound concern regarding the decision of the C.I.U.S.S.S. to close the 388 (Center for Treatment of Young Psychotic Adults) in Quebec City. This decision poses a significant risk to the mental health landscape, both locally and globally, and is sure to have negative effects on those most in need of such services.
The 388 is a vital institution, at the cutting-edge of the treatment of psychosis. Its integration of psychoanalysis with a community-based framework and a multi-modal, interdisciplinary approach serves as an inspiration for many of us. In Chicago and Boston, New York and Los Angeles and beyond, many of us have explored and continue to look to the treatment model developed at the 388 as a source of inspiration and guidance in the development of clinical offerings for patients in our own communities who suffer from psychosis. As mental health clinicians working on the frontlines, we recognize the unique value of the 388’s approach in responding to the needs of patients who so often slip through the cracks of existing service frameworks. This treatment offers a rare and essential opportunity for the suffering person not only to manage their symptoms, but to explore their humanity, engage in deep transformation, and find new forms for their contributions as citizens in the world with others. This is not merely a treatment facility—it is a sanctuary where individuals suffering from profound difficulties can rebuild their lives, as many of its users have attested. The closure of the 388 would dismantle a deeply rooted and irreplaceable model of care that addresses the complexities of psychosis in ways that few, if any, other facilities do.
Many of the undersigned have regularly traveled hundreds or even thousands of miles in order to learn about the psychoanalytic approach of the 388, and to participate in training seminars offered by GIFRIC. This education has been vital to our own clinical practices. The teaching and leadership of analysts Lucie Cantin, Danielle Bergeron, and Willy Apollon have had a generative effect, inspiring organizations and practitioners far beyond Quebec. The intellectual and ethical rigor that characterizes the 388 has been an inspiration for leading clinicians, academics, and educators not only in the field of psychoanalysis but also in the fields of psychiatry, clinical mental health counseling, clinical psychology, anthropology, and social work. The 388 has been a site for research and is an exemplary clinic, with data-supported results for the treatment and long-term re-integration of young people with psychosis into the local community as contributing members of society. It is difficult to overestimate the truly singular impact the work at the 388 has had on seeding new ideas in mental health treatment and research.
The closure of the 388 would not only dismantle a singularly effective model of care but also represent a significant setback for the global mental health community, which looks to this institution for guidance and innovation. We implore decision-makers to engage with stakeholders, including clinicians, patients, families, and the community, to explore alternatives that preserve the invaluable work being done at the 388. By ensuring its continued operation, you will be safeguarding a beacon of hope for individuals with psychosis and fostering the advancement of mental health care globally. Let us work together to sustain and expand the legacy of the 388, rather than allow it to become a cautionary tale of lost potential. We urge you to reconsider this decision.
